Infecciones Respiratorias. Gripe. | Responsable: Rafael Zalacain Jorge

Las enfermedades crónicas suponen ya, y van a suponer en un futuro muy próximo, cada vez una carga mayor para la sociedad y el sistema sanitario. La EPOC es un proceso altamente prevalente que ya supone un alto coste especialmente en lo referente a las hospitalizaciones. La telemedicina se ha demostrado útil para el control de otras enfermedades. También en la EPOC puede ser un instrumento que ayude a un adecuado y coste-efectivo control de estos pacientes. Se propone un estudio de cohortes, longitudinal, prospectivo pre y post-intervención, coordinado y multicéntrico con grupo de intervención (sub-proyecto 1) y grupo control externo (sub-proyecto 2). Al cabo de un año el grupo control pasará tambien a formar parte del grupo de intervención. Se recogerán datos retrospectivamente del año anterior al comienzo del estudio en ambos centros. Se medirán variables clínicas y fisiológicas, de calidad de vida y satisfacción del paciente al inicio, al cabo de un año y al final del estudio. Se pondrá en marcha un dispositivo de telemonitorización a distancia, recogiéndose diariamente una serie de variables fisiológicas y un cuestionario clínico. Se establecerá un sistema de alertas y de respuestas. Estas respuestas estarán coordinadas entre la asistencia priMaría y la especializada. Resultados: ingresos hospitalarios, complicaciones, valoraciones en servicios de urgencias, evolución de la calidad de vida, satisfacción del paciente y costes en ambos grupos. Análisis estadístico: se compararán los resultados a medir entre los dos grupos (intervención /control) y entre antes-después de la intervención en cada centro. En el análisis multivariante se emplearán modelos de regresión logística y la regresión de Cox condicional. Para la comparación pre-post se emplearán modelos GEE.

 

Enfermedad Tromboembólica Venosa. Hipertensión Pulmonar | Responsable: Fernando Uresandi Romero

La neumonía adquirida en la comunidad (NAC) con una incidencia anual que oscila entre el 5 y el 11% de la población adulta constituye  una de las principales causa de ingreso hospitalario en los países occidentales especialmente entre los pacientes ancianos con múltiples comorbilidades. La mortalidad asociada a esta entidad en los pacientes ingresados oscila entre el 5 al 50% dependiendo de la gravedad del cuadro.

En nuestro hospital una media de 350 pacientes ingresan anualmente en el Servicio de Neumología por esta entidad que representa la tercera causa de ingreso hospitalario urgente. Aproximadamente un 11 % de estos pacientes van a requerir de asistencia en el Servicio de Medicina Intensiva.

En los últimos años nuestro grupo ha venido participando en la elaboración y desarrollo de varios proyectos multicéntricos relacionados con el tema y que ha dado como fruto la publicación de varios artículos originales en revistas de reconocido prestigio internacional. Actualmente estamos interesados en el estudio de diversos aspectos relacionados con la epidemiología de las NAC bacteriémicas así como en el papel que las diferentes comorbilidades tienen en la presentación clínica y en el pronóstico de los pacientes ingresados con esta entidad.

Durante el año 2009 y coincidiendo con la epidemia por el virus A1H1 nuestro Servicio ha participado en el Proyecto de Investigación de la gripe estacional 2010-2011 patrocinado por el Instituto de Salud  Carlos III, Ministerio de Ciencia e Innovación (MICINN), CIBERES, SeMycuc ( Sociedad Española de Medicina Intensiva y Unidades Coronarias).

 

Patología Respiratoria del Sueño. Insuficiencia Respiratoria. Vmni | Responsables: José Amilibia Alonso

El Síndrome de Apnea-Hipopnea del Sueño (SAHS) es una entidad muy prevalente, que puede llegar a afectar hasta el 5% de la población, y que ocasiona en los afectados un grave deterioro de su calidad de vida. Además está relacionado con una importante morbimortalidad cardiovascular y con la génesis de accidentes de tráfico. La carga sanitaria y económica que todo esto implica se ve aún más potenciada por los costes que genera su correcto diagnóstico (asociado con frecuencia a considerables listas de espera) y tratamiento (principalmente el uso de CPAP). Nuestro grupo está especialmente interesado en profundizar en aspectos eminentemente prácticos que permitan mejorar, simplificando y haciéndolo más ágil, el proceso diagnóstico y terapeútico de esta enfermedad, así como en el desarrollo y correcta indicación de otras posibilidades terapeúticas (terapias posturales, cirugía, dispositivos de avance mandibular…).

Por otro lado también estamos trabajando en conseguir un conocimiento más detallado de las implicaciones cardiovasculares que este síndrome implica, y de las posibilidades terapeúticas que de ellas se derivan en patologías de tanta importancia como el Síndrome Coronario Agudo.

Nuestro grupo ha desarrollado también una importante actividad relacionada con la Ventilación Mecánica No Invasiva, modalidad terapeútica de crecimiento exponencial en el campo de la Neumología en los últimos años. Esta actividad se ha producido tanto en lo que atañe al tratamiento de la Insuficiencia Respiratoria Hipercápnica Crónica de diversos orígenes y a los trastornos respiratorios del sueño asociados (hipoventilación alveolar nocturna…), como en lo que respecta al tratamiento del Fracaso Ventilatorio Agudo.

Finalmente debemos mencionar nuestro interés en la aplicación de otras técnicas novedosas para el manejo de la Insuficiencia Respiratoria Hipoxémica, como puede ser la CPAP o la Oxigenoterapia de Alto Flujo, con humidificación activa.

 

Fibrosis Quística | Responsables: Baranda F, María Dolores Pastor Vivero, Gómez Bonilla A, Gómez Crespo B.

Los miembros de la Unidad de fibrosis quística (FQ) del Hospital Universitario de Cruces tienen acreditada una dilatada experiencia clínica e investigadora en los distintos aspectos de la enfermedad como son el Screening, protocolo diagnostico, control evolutivo y terapéutica de los pacientes afectos de FQ de más de 30 años. El HU de Cruces es el hospital de referencia de esta enfermedad para la totalidad del País Vasco, Navarra, Cantabria y Castilla-León. En la actualidad la unidad controla un total de 185 pacientes.

El Dr. Félix Baranda, responsable de la Unidad FQ de adultos y la Dra. María Dolores Pastor Vivero, responsable de la Unidad de FQ pediátrica; forman parte activa de las Sociedad Española de Fibrosis Quística participando en los consensos de diagnóstico y tratamiento.

En la Unida FQ de adultos, se controlan paralelamente  un grupo de 40 pacientes afectos de Bronquiectasias no FQ colonizados por microorganismos multirresistentes y que requieren de una valoración diagnostica, seguimiento y tratamiento multicomplejo específico. La experiencia en esta área de la Neumología queda reflejada en la elaboración de las monografías del diagnóstico y tratamiento mediante terapia antibiótica inhalada.
